PHARMACODYNAMICS:
Pharmaceutical effects of the drug is a effective combination of two components and the additional effect of the two components.
Perindopril:
Anti-hypertensive effect: Perindopril effects with all levels of hypertension, lowering systolic and diastolic in both standing and on back positions. Perindopril has vasodilatory properties, restore the elasticity of large arteries and reduces left ventricular hypertrophy. Anti-hypertensive effect reached a maximum after 4 - 6 hours and maintained over 24 hours.
Effects on heart failure: Perindopril reduces the load for the heart by relaxing veins and reduces the total peripheral resistance. So drugs are effective for reducing filling pressures in the left atrium and right atrium, increasing cardiac output, increasing the myocardial region blood flow and improving cardiac index.
Indapamide:
Indapamide is a sulfonamide derivative with structure of indole, a pharmacological relationship with the group of thiazide diuretics.
The drug has diuretic action and anti-hypertension by inhibiting sodium resorption in the renal dilution of renal shell, increasing excretion of sodium, chloride and a part of potassium - magnesium, thereby increasing urinary excretion and lowering blood pressure. Beside, the drug also effects to reduce left ventricular hypertrophy.
Anti-hypertensive effect was achieved even at doses that diuretics is very mild. When used separately, the drug effects lasting for 24 hours.
PHARMACOKINETICS:
Indapamide:
Rapidly and completely absorbed from the gastrointestinal tract. Food or antacids almost no effect on the absorption of the drug.
After administration, peak blood concentrations achieved in about 1 hour, about 79% of the drug bound to plasma proteins.
Half-life about 14 - 24 hours (average 18 hours). Indapamide was metabolized strong in the liver, mainly into sulfate and glucuronid conjugate. Hemodialysis does not remove indapamide from circulation.
Perindopril:
After administration, about 65 - 70% perindopril absorbed (reaching peak concentrations approximately 1 hour) and metabolized mainly in the liver into active metabolites as perindoprilate and some other inactive metabolites.
Half-life about 1 hour. About 20% was converted into biological perindoprilate.
Food reduces the biological convertion into perindoprilate and thereby reducing bioavailability. Drugs associated with less protein, less than 30% but depends on the concentration.
Perindoprilate was excreted through urine and excretion time about 3 - 5 hours. Excretion of perindoprilate was reduced in humans with heart failure, kidney failure. Both perindopril and perindoprilate were eliminated by dialysis.
INDICATIONS:
For treatment of primary hypertension patients but uncontrolled with single therapy by perindopril.
DOSAGE AND USAGE:
How to use: oral route, should take in the morning before eating.
Dose: Usual dose is 1 tablet per day (single dose).
If drugs are not responded, could increase the dose to maximum of 2 tablets per day.
Renal failure: in patients with severe renal impairment (creatinine clearance < 30 ml/ min): drugs was contraindicated.
In patients with renal impairment and creatinine clearance > 30 ml/ min and < 60 ml/ min: recommended dose of perindopril was 2 mg per day. Therefore, do not use Covaprile plus that must be used other drugs.
In patients with creatinine clearance > 60 ml/ min: no need to change the dose

    CONTRAINDICATIONS:
Hypersensitivity to perindopril, with sulfonamid or any component of the drug.
There was a history of angioedema (Quincke oedema), untreated non-compensation heart failure.
Severe renal failure, severe liver failure.
Decreased potassium blood disease, liver disease brain.
Women during pregnancy or lactation, children.
Not recommended in cases: combination with potassium sparing diuretics, potassium salt, lithium, in people with renal artery stenosis or hyperkalemia.
PRECAUTIONS:
The drug should be used cautiously in patients following:
There was electrolyte balance disorders, diabetes, low blood pressure or are applying salt diet strictly.
Heart failure or kidney failure, atherosclerosis, renal artery stenosis.
Elderly people, people with impaired liver function.
History of face oedema, lips oedema or throat oedema.
Hemodialysis.
INTERACTIONS:
To avoid interactions, must be fully informed all therapies to your physician or other therapies are used most recently.
Should not be combined with lithium can increase the concentration of lithium in the blood to toxic levels.
Caution in combination with potassium salt, potassium sparing diuretics, the drugs can cause arrhythmias, anesthetic, allopurinol, cell inhibitors or immunosuppression, corticosteroids, other anti-hypertensive drugs can increase the effect of hypotension.
ADVERSE EFFECTS:
Feelings of fatigue, dizziness, headaches, sleep disturbances, cramps, low standing blood pressure, skin reactions on the type of allergies, hives, stomach pain, loss of appetite, nausea, abdominal pain, constipation, taste disturbances.
Dry cough, dry mouth, increased risk of dehydration in elderly patients and patients with heart failure.
Rare: angioedema, pancreatitis.
Notify your physicians if you should experience any of adverse effects
EFFECTS ON ABILITY TO DRIVE AND USE MACHINES:
Because drugs can cause dizziness should be especially cautious when driving or operating machinery.
PREGNANT WOMEN AND LACTATION:
Do not use for pregnant women or breast feeding.
OVERDOSE AND TREATMENT:
The most common symptoms in cases of overdose is hypotension, nausea, vomiting, cramps, dizziness, balance disturbances of water and electrolytes.
Treatment: can use measures as gastric washing or take activated charcoal, supplement of water and electrolytes to increase the volume of blood if necessary.
If hypotension occurs significantly, can be treated by put patients on back with lower head and higher feet.
Perindoprilate may be removed by hemodialysis.
STORAGE: in a dry place, below 30oC, protect from light.
SHELF-LIFE: 36 months from the date of manufacturing. Do not use after the expiry date.
PACKAGING: Box of 2 blisters x 30 tablets.
